I acknowledge the contagious nature of the COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
NUWS cannot prevent you or your team from becoming exposed to, contracting, or spreading COVID-19 while participating in NUWS activities. It is not possible to prevent against the presence of the disease.
Therefore, if you choose to participate and your team chooses to participate in NUWS activities, you and/or your teammates may be exposed to an increased risk of contracting or spreading COVID-19.
I have read and understood the above warning concerning COVID-19.
I hereby choose to accept the risk of contracting COVID-19 for myself and/or my team members in order to participate in NUWS activities. These activities are of such value to me and/or to my team members that I accept the risk of being exposed to, contracting, and/or spreading COVID-19.
This waiver should be completed by each manager registering a team who is choosing to play soccer with NUWS and wish to participate in NUWS activities.
I and my heirs hereby forever release and waive my right to bring suit against NUWS and its officers, employees, officials, agents, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to participating in NUWS activities. I understand that this waiver means I and my heirs give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence, and I and my heirs give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
I agree that all members of my team will abide by the rules of the Northwest United Women's Soccer League (NUWS), the United States Soccer Federation (USSF), the United States Adult Soccer Association (USASA), the Oregon Adult Soccer Association (OASA) and the affiliated leagues to which my team belongs. I accept full responsibility for my team and understand that I am responsible for the conduct of my team and its fans and the financial obligation of my team to pay its fees, fines and debts. I agree that if my team is providing its own field and that if I do not submit complete field availability and use information by the registration deadline and/or the field is not available for use under the guidelines provided by NUWS, my team will be assigned a field by NUWS and my team will be required to pay the field use fee for this season.
The following information is a guide for the NUWS 2021 Spring season. This document includes information on NUWS requirements for teams to participate in the 2021 Spring season. The goal of NUWS is to ensure the health and safety of all participating members, while moving forward with league play. The goals and requirements of this document are subject to change based upon directives from the Governor’s office and NUWS board discretion.
As part of your registration for the upcoming season, each manager must sign the waiver below.
Each team must use the updated roster form provided by NUWS for all games. This new roster requires each player to read the COVID-19 screening questions and sign that they are cleared to play the game, as well as acknowledge the assumption of risk by participating in NUWS activities. This information is vital should contact tracing be needed. Therefore, the new roster form must be:
Completed prior to every game and captured digitally prior to providing the physical document to the referee
Sent as a digital copy to your Division Rep after each game; update this digital copy with any late arrivals information
Updated with any late-arriving players, who must present their OASA card and sign the roster prior to playing in the match
Filled out and signed by a legal guardian for any minors participating on their behalf.
No player should attend a game if they have known exposure or are experiencing COVID-related symptoms.
All players who are not fully vaccinated, and any unvaccinated spectators when distancing cannot be maintained, must always wear a face covering on and off the field. Fully vaccinated players may opt out of wearing a mask. Masks should be properly worn, covering nose and mouth at all times. CDC mask guidelines should be followed. Unacceptable face coverings include:
face shields
bandanas
masks with exhalation valves/vents
neck gators
Each manager is responsible for their team’s adherence to these requirements. Referees may assist but will not be required to ensure adherence to the league’s mask mandate. Managers and teammates are responsible for holding each other accountable. We have confidence that the desire to play soccer will override any potential conflicts pertaining to mask-wearing.
Should a team decide it is unsafe for them to play an opponent due to COVID-19 issues (e.g., non-mask wearing, COVID-19 symptoms of an opponent) and every effort has been made to resolve the conflict, teams may opt-out of that game. Managers should report the incident to their Division Rep as soon as possible and NUWS will investigate the issue. If it is determined that a team blatantly disregarded the requirements in this document and the roster form, that team may be penalized with a forfeit. The forfeit rules in the NUWS League Rules will apply.
Managers should be alerted if any player on their team tests positive for COVID. If that player has played in a NUWS-league game within 72 hours of the onset of symptoms, the manager must immediately notify their Division Rep. The next 2 games for that team, and their opponent will be cancelled by NUWS. Cancellations due to COVID-19 concerns would not be considered a forfeit and there will be no fines issued. That player may return to play after 14 days from the positive diagnosis, and they are symptom-free.
We are one community and we rely on each other to be honest and caring around these rules just stated. Playing the game is not worth the chance of infecting another team.
It is imperative that managers keep the team roster accurate. This document will be used to aid the manager and league in performing contact tracing. Should a manager know or suspect an infection, the manager must:
Isolate and send the person home immediately
Immediately notify your Division Rep and keep them apprised as this issue progresses; the Division Rep will notify the opposing manager(s) as appropriate
Immediately notify all of the players that came in contact with the infected player
Follow any other State guidelines on contact tracing
It is possible that subsequent games may be cancelled at NUWS discretion.
In addition, managers must capture information on the roster form for their spectators. There is a section to have spectators provide their names, phone numbers, and COVID-related acknowledgements. This information does not need to be provided to the Division Representative, but should be kept on hand for 60 days afterwards.

I further acknowledge that NUWS has put in place preventative measures to reduce the spread of the COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while participating in these activities, and as the team manager, must communicate all procedures to my team and ensure they adhere to them to the best of my ability.
