Submit Referee Evaluation

Please fill out an evaluation for each game that you play. Evaluate the referee for each of the following categories, and add any additional comments as you feel necessary. Misinterpretation of the Laws of The Game or the NUWS League Rules & Bylaws should be noted. Please allow enough time after the game so that you can give us an objective perspective of the job the referee did, but please get them in within two weeks of the game date. Thank you for your time, this information can help us in giving feed back to the referees, or weeding out those who do not meet our standards.
